Depending on your province of residence and subject to applicable privacy and professional requirements, you may be entitled to obtain access or ask to modify/correct the personal information we hold about you. You are also entitled to withdraw your consent to the use or disclosure of your personal information at any time.

Consent to Care: By accessing and using the Platform to access healthcare services, you consent to treatment by our healthcare professionals. Healthcare professionals may ask you questions about your health history, present condition or symptoms, and any course of treatment or medication that has been prescribed to you. They may also conduct a visual examination of you virtually, and ask you to undergo additional assessments such as tracking your symptoms, collecting samples or undergoing tests.  In prescribing or communicating any diagnosis, course of treatment or medication to you, the Healthcare Practitioner will seek your informed consent which requires the Practitioner to explain the diagnosis, treatment and risks and side effects associated therewith. You may withdraw your consent to care at any time by ending your consultation with the healthcare professional.

 

Risks and Limitations of Virtual Care: As a registered member, you may have access to Virtual Care services. Virtual Care refers to providing or assisting with patient care at a distance using information and communication technologies, including through the App. While there are many benefits to this approach, Virtual Care also presents inherent risks and limitations, including:

  • Inability to Handle Serious Conditions: Some medical conditions require immediate in-person care and may not be appropriately treated via telemedicine. IF YOU BELIEVE YOUR CONDITION REQUIRES IMMEDIATE ATTENTION, PLEASE CALL 911 OR YOUR LOCAL CLINIC.
  • Poor audio-video quality: Remote locations or network issues can affect the quality of the exchange with health care providers.
  • Delays in treatment: Technology failures may cause delays in evaluation or treatment.
  • Incomplete assessment: Healthcare providers may miss signs or symptoms that can only be detected through in-person examination, potentially leading to incomplete or inaccurate diagnoses.
  • Limited access to health records: Not having access to all your personal health records could lead to medication errors or allergic reactions. Always inform the healthcare provider of any medications or allergies.
  • Security risks: Unauthorized access or breaches of your personal information (PI) or personal health information (PHI) may occur due to security failures.

 

Consent. By creating an account with Coral, you :

  1. Confirm your understanding of the risks and limitations associated with your use of our the Coral Platform and all the available services in the App, as summarized above and more fully described in our Terms of Service;
  2. Agree to the collection, use and disclosure of your personal information, including your personal health information, as outlined in our Privacy Policy;
  3. Consent to receive Services from Healthcare Providers and Wellness Providers;
  4. Agree to be bound by our Terms of Service.
